Optimising NDIS plans into real world support
Our Support Coordinators help you get the most out of your NDIS plan—connecting you with services, handling tricky admin, and advocating for what you need. Services include (but not limited to):
- Understanding your NDIS plan and what’s included
- Finding and connecting with the right service providers
- Coordinating care between teams (e.g. therapists, support workers)
- Communicating with the NDIA on your behalf
- Navigating challenges or changes to your plan
- Preparing for plan reviews, including gathering the right reports and evidence to support plan increases or changes
- Identifying gaps in your current supports and unmet needs
- Helping you understand what evidence is needed for NDIA decisions
- Building skills to manage your own supports more independently
Support Coordination is funded in many plans, depending on your needs—check your plan or speak with our team.
As Support Coordinators we need to be able to balance our expertise and knowledge in our role, with the consideration for the participants whole journey, not just on the NDIS. This means using empathy and understanding to meet them where they are on their journey, and then supporting in the manner and ways they wish to be supported.”


Case study: A journey to independence
After months of support from our Support Coordinator, a young client successfully transitioned to independent living. Navigating the SIL /SDA process and choosing the right home, our Support Coordinator worked closely with his mother to prioritise his needs and wishes.
Emotional support was just as important—especially for his mother, adjusting to him leaving home. “I’d be lost without Mark—he genuinely cares,” she shared. After 8 months of coordination, paperwork and visits, he’s now happily settled in.
